An aged care provider working under the Support at Home program is hiring a Care Partner / Case Manager in Sydney, and the ideal person is compassionate and experienced, with the confidence to work autonomously in a flat-structure, close-knit group.
You would be the point of contact for clients, steering care coordination, managing individual support plans and checking that the right services arrive when they should. The provider says visa sponsorship is also available for the right candidates.

Key Responsibilities
- Work respectfully with clients, families, support workers and outside providers to form strong relationships
- Line up services around clients' goals and health conditions
- Assess clients, then draft and update care plans
- Let client safety, wellbeing and quality of life guide your decisions
- Supervise the Support Worker team and keep service continuous and good
- Manage client budgets
- Comply with legislation, funding guidelines and policy, write progress notes on time, read colleagues' and providers' notes, and carry out intake assessments for new clients
Requirements & Eligibility
- Past work in aged care, home care, community services or health
- Skills in assessment, care planning and case management
- Confident working alone and handling competing priorities
- Excellent communicator, relationship builder and leader
- Working knowledge of My Aged Care and the Aged Care Quality Standards
- Driver's licence and own vehicle, with willingness to travel across Sydney
- Bilingual or multilingual ability is a strong advantage; a clinical background such as nursing or allied health is beneficial but not required
